If you are interested in a free e-mail account from www.cog-onlinestudy.com you will need to fill out the form below. 

Step one:  Enter First Name, Last Name, Location (State), & Check Gender.

Step two:  Choose a e-mail address (Up to 15 characters long).  Make sure the username you create contains no spaces.  And please include alternate e-mail for notification of new account completion.

Step three:  Create a private password (Up to 8 characters long).  Re-enter for password verification

Step Four:  Enter additional information/questions and then click the submit button.  Please allow 24 hours to process your request.  A notification will be sent by e-mail once your new e-mail address has been activated. 

Note:  Your information will be kept in a registration database that is accessible only to the webmaster.
